Joining as a California district (Exhibit E piggyback)

Under Exhibit E of the executed agreement, other California local education agencies may adopt the same terms by referencing the originating agreement — no full legal renegotiation required. Your procurement team completes a standard joining form through your CITE Privacy Services process.

What we send within one business day of your request: (1) the fully executed CA-NDPA copy, (2) Exhibit E joining exhibit, (3) our W-9, (4) a district quote. Email lidaban2025@gmail.com with your district name.

What FormGuard does and does not access

Does: reads and updates response-acceptance settings, question options, and its own configuration stored within each form; sends optional alerts to the form owner's own email.
Does not: read, transmit, or retain form responses or any student personal information; make external network calls from the add-on.
